JotForm is a web based WYSIWYG form builder service, used to build complex forms. Using JotForm, you can create forms, integrate them to your site and receive responses via email. For more information, see JotForm FAQ.
Using a JotForm service, you can let a site visitor upload documents, build surveys, event calendar, and more. Generally, creating and embedding such a form within a web page is a complex process, but its 3-step process with the new JotForm widget.
Download JotForm Widget

Click the Get This Widget button.
The JotForm widget is downloaded to your default Downloads folder. Depending on the browser you used to download the widget and the operating system on your computer, the Downloads folder is configured differently.
Add and configure a complex form
To add a complex form to a web page, do the following:

In Adobe Muse, select Window > Library to open the Library panel. If you haven't already done so, click the
button, and import the JotForm Widget you downloaded. For information about downloading the widget, see Download JotForm Widget. -
In the Library panel, select JotForm > JotForm, and drag the widget on to the web page canvas.

Select the JotForm widget, and click
button to open the Options panel.

In the Options panel, enter the following:
JotForm ID
Provide unique ID of your JotForm form.
Allow Form Scrolling
You can choose to enable scroll-bar on the form. The Allow Scrolling option is useful if you have a large form that needs to fit in a small space, this allows site visitors to scroll within the form frame.

Click outside the Options panel to finish. Preview the web page or the site using Preview mode in Adobe Muse or select File > Page in Browser option.
