Create PDF forms from scratch

Learn how to create a fillable PDF form starting with a blank page in Adobe Acrobat.

You can create a fillable form from scratch in Adobe Acrobat. You can start with a blank PDF, add the required text and labels, add form components, and then adjust the layout and formatting of your form fields.

Select Menu (Windows) or File (macOS) > Prepare a form.

In the Prepare a form window, select Start from blank page.

If you want to others to sign the form, select This document requires signatures.

Select Create form. A blank PDF form opens in editing mode.

To add text, select Text under ADD CONTENT in the Prepare a form pane. Select the spot in the page and enter the information.

To add more form fields, select the desired components, such as an Image Fielda Checkbox, or a Radio button, under ADD FORM COMPONENTS in the left pane.

The Add Form Components pane is visible with options including Text field, Image field, Checkbox, and more.
To add components, select the option from the ADD FORM COMPONENTS pane.

To delete a form field, right-click the field and select Delete.

Select Preview to see how your form will appear to the recipients and save the form. 

Tip

After saving the form, you can use the Send button to distribute your form and collect data.
